Patient Information

We believe your time is as valuable as ours. To make your time here as efficient as possible, we encourage you to prepare your Health History form and Patient Sign Off form before your appointment. If you need assistance with the form or have other questions, please call the office at 978-531-0800. Our Notice of Privacy Practices is available here online as well. We look forward to your visit.

We are pleased to provide a patient portal so you can communicate non-urgent messages to us at your convenience, such as appointment requests and prescription refills. The portal link is below.

We strive to provide the best orthopedic care in New England. Your complete satisfaction is important to us. If there is anything we can do to improve, please contact our Practice Administrator at 978-818-8218. You may receive a Patient Satisfaction Survey via text or email from a service called Rater 8. You are also welcome to fill out our Patient Satisfaction Survey and mail it to our main office in Peabody or drop it off at either one of our offices. We welcome and appreciate your feedback.

We recommend that you wear loose clothing and no jewelry for your visit in case it is determined that an x-ray is needed.
We are currently using a “virtual” waiting room. When you arrive in the parking lot for your visit, please call us on our special check-in phone line at 978-818-8233 for the Peabody office . Once we check you in over the phone, we ask that you wait in your car until the clinical team has a room ready for you and calls you in. Please be sure to give us the number for the cell phone that you have with you.
We will be screening you for the COVID-19 virus on the day of your visit and will be taking your temperature with a “no-touch” thermometer. Please note that a face covering over your mouth and nose is required to be worn during your entire visit.
We have put several processes in place to keep our patients and our staff safe. Please see the notice below about “How We Keep You Safe”.
